Achondroplasie im Überblick

Die Achondroplasie ist die klinisch häufigste Skelettdysplasie aus der Gruppe der Skeletterkrankungen. Weltweit sind mehr als 250.000 Menschen von Achondroplasie betroffen.1,2 In Europa wird die pädiatrische (< 18 Jahre) Prävalenz der Achondroplasie auf 3,72 Fälle pro 100.000 Geburten geschätzt. 3

Achondroplasie ist eine seltene genetische Erkrankung, die durch eine systemische Variante des Fibroblasten-Wachstumsfaktor-Rezeptors 3 (Fibroblast Growth Factor Receptor 3, FGFR3) verursacht wird. Sie führt zu einer gut charakterisierten Skelettdysplasie sowie zu schwerwiegenden muskulären, neurologischen und kardiorespiratorischen Komplikationen.1,2,4–9

Bei Achondroplasie führt die konstitutive Aktivierung von FGFR3 in Chondrozyten zu einer anhaltenden mitogen-aktivierten Proteinkinase-Signalübertragung (Mitogen-Activated Protein Kinase, MAPK). Dies hemmt die Chondrozytendifferenzierung und resultiert in einem verminderten enchondralen Knochenwachstum sowie in einer Skelettdysplasie.10–12

Eine zentrale klinische Manifestation der Achondroplasie ist dysproportionierter Kleinwuchs. Darüber hinaus sind die Auswirkungen nicht auf das Knochenwachstum begrenzt: Auch muskuläre, neurologische sowie kardiorespiratorische Komplikationen können auftreten.7 

Die Behandlung der Achondroplasie umfasst sowohl pharmakologische Therapien als auch chirurgische Interventionen. Zugleich besteht weiterhin ein medizinischer Bedarf an zielgerichteten Interventionen, die klinische Manifestationen verbessern und Achondroplasie-assoziierte Komplikationen gezielt adressieren können.6,10 Moderne Therapieansätze zielen darauf ab, die FGFR3-Funktion und die MAPK-Signalwege zu beeinflussen oder direkt auf Chondrozyten zu wirken.10
